Center Street Community Health Center

This position involves performing vision tests, diagnosing eye problems, and prescribing treatments like eyeglasses and medications. Key responsibilities include conducting daily eye exams, checking for vision issues or diseases (e.g., glaucoma), addressing patient concerns, and educating on eye care options. The role also requires analyzing test results, prescribing corrective lenses or medications, maintaining equipment, and working with staff to meet patient needs. Additionally, the position involves recording patient data, participating in training, and referring patients to specialists when necessary.

*Doctor of Optometry (O.D.) required from an accredited educational institution
*Current state optometry license, valid and in good standing
*Minimum two years experience working as a practicing optometrist
*Solid working knowledge of all ophthalmic equipment needed to conduct examinations

Family Practice Physician

Center Street Community Health Center

The Family Practice Physician provides comprehensive, patient-centered primary care to individuals and families across all ages. Responsibilities include diagnosing and treating acute and chronic conditions, managing ongoing diseases, delivering preventive care, performing minor in-office procedures, providing women’s health services, and prescribing and monitoring medications while collaborating with the care team.

*Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
*Completion of an accredited Family Medicine residency program
*Board-certified or board-eligible through the American Board of Family Medicine
*Active, unrestricted state medical license
*DEA registration
*Current BLS and ACLS certification


Dentist

Morrow Family Health Center, Mt. Gilead

The dentist is responsible for providing general dental care for the patients of Galion Family Health Care. The dentist would supervise, direct, train and educate dental ancillary staff to perform their duties. The dentist will provide preventative and restorative care including cleanings, x-rays, exams, fillings, root canals, crowns, partials, and dentures. The dentist will maintain an affiliation with the professional groups, dental associations, and dental organizations, as per policies and procedures established by Center Street Community Health Center and represent CSCHC at all meetings of these associations and organizations whenever possible or as directed by the Associate Dental Director. Provide “emergency call” as scheduled by the dental director in accordance with protocols established in that regard and common and standard practices. Participation in community health events, such as school screenings. Must become familiar with Federal, State, and local funding agencies’ rules and regulations in providing special dental services to certain classes of recipients of services. Become familiar with Federal and State dental programs and their requirements.

* Graduate from accredited Dental School
* Must have Ohio Dental license
* Authoritative knowledge of the practice and techniques of dentistry as a general practitioner in a community health center is highly desirable, but not required


Family Nurse Practitioner

Center Street Community Health Center, Marion

The Nurse Practitioner is responsible for providing medical services for the patients of Center Street Community Health Center within the scope of the practice according to the requirements of the Ohio Board of Nursing, Written protocol, and ability. The Nurse Practitioner may be directed by the clinical Director to perform other duties in accordance with funding policies, procedures, and protocols.

*Master of Science Degree in Nursing.
*Current License to practice Advanced Nursing in the State of Ohio

*Current Basic Life Support training


Dental Assistant

Center Street Community Health Center, Marion

The dental assistant is responsible for assisting the Dentist during examinations, procedures, and treatments. They also reassure and comfort patients, deliver postoperative instructions, and counsel patients and/or parents on proper oral hygiene and plaque control. They will also sterilize dental instruments and equipment, place the items for easy access, prepare the dental exam room for use by the dentist by making sure it is stocked and organized, assist the dentist during procedures, including passing instruments (using the concepts of four-handed dentistry), getting supplies, and using the suction tube to keep the patient’s mouth dry. The dental assistant will also take digital dental x-rays.


*High School Diploma or GED
*Radiographers certification
*CPR/BLS certified
*Previous experience preferred, but not required


Dental Hygienist

Center Street Community Health, Marion and Galion

The dental hygienist works under the direction of the dentist. They participate in the assessment, preparation, and delivery of patient care within the federal, state, and institutional regulations and guidelines for the provision of dental outpatient services. Responsibilities and duties include but are not limited to room and equipment preparation, patient screening, charting and medical history documentation, ability to assist and execute a wide range of procedures and techniques, and participating in emergency care when required.

Licensed Practical Nurse

Morrow Family Health Center, Morrow

The LPN works under the direction of the Clinical Services Manager. They participate in the planning, implementation, and evaluation of nursing care, within the objectives, standards, and policies of the health center and within the parameters of the scope of practice for the Licensed Practical Nurse. Responsibilities and duties include but are not limited to direct and individualized nursing care to patients, chart prep, follow up of outstanding orders and tasks, health education for patients and family, preparing and administering medications, participating in emergency care when required, communicating patient needs with other medical providers and ancillary personnel as directed by plan of care such as referrals, medication plan, and scheduling of tests. The LPN must record information concisely, accurately, and completely.  
*LPN – Licensed Practical Nurse – National Council of State Boards of Nursing 
*Graduate from an accredited school or program of practical nursing.  
*Must hold a current license from the State of Ohio.

Optometric Technician

Center Street Community Health Center, Marion 
Work with our patients to deliver quality optical care and outstanding, friendly customer service. Assist optician and optometrist with testing, preparing exam rooms, and other support activities. Perform duties as a receptionist as needed to provide consistent patient care. Meet with patients to document prior eye care history, determine current prescription, and establish a patient record in the EHR.
*High School graduate or equivalent 
*Prior work or education as MA or STNA is helpful
*Focus on attention to detail 

Pharmacist, PRN

Center Street Community Health Center, Marion
The Pharmacist is responsible for reviewing, preparing, and dispensing medications to support comprehensive, patient-centered care. The pharmacist promotes safe, effective, and affordable medication use, supports access to care for underserved populations, and ensures compliance with clinical standards and 340B program requirements. The pharmacist collaborates with providers and care teams to advance the health center’s mission and improve patient health outcomes.
* Active Ohio Pharmacist license in good standing (required)
* Doctor of Pharmacy (PharmD) preferred; Bachelor of Pharmacy accepted for pharmacists licensed prior to the PharmD requirement.
* Strong interpersonal, communication, and customer service skills.
* Strong organizational and time management skills.
